How this list was compiled: Our own questions on health assessment, written to the 2026 NCLEX-RN Test Plan of the National Council of State Boards of Nursing (https://www.nclex.com/files/2026_RN_Test%20Plan_English-F.pdf, linked from https://www.nclex.com/test-plans.page, effective April 1, 2026 through March 31, 2029, opened and read 2026-10-12): under Health Promotion and Maintenance, Health Screening (health history, risk assessments, interviewing techniques) and Techniques of Physical Assessment (psychomotor skills, equipment and technique suited to the client, comprehensive health assessments); under Reduction of Risk Potential, System-Specific Assessments (peripheral pulses, neurologic status, peripheral edema, focused assessments) and Changes/Abnormalities in Vital Signs. The plan gives these headings no shares of their own, so the split is this page's: health history and interview 7, general survey and vital signs 11, examination techniques 8, heart and circulation 9, lungs and breathing 9, abdomen 5, neurological assessment 8, skin, vision and hearing 5. Every fact was read on 2026-10-12 in the public page the question names: the open textbook Nursing Skills 2e (WisTech Open, CC BY 4.0), and for one question Nursing Fundamentals 2e of the same publisher. How it works

These 62 free health assessment NCLEX questions are our own: history taking, vital signs, inspection, palpation, percussion and auscultation. They are unofficial, not affiliated with, endorsed by or sponsored by the National Council of State Boards of Nursing (NCSBN).

Where assessment sits in the test plan

The 2026 NCLEX-RN Test Plan, read October 12, 2026, puts health history, screening and techniques of physical assessment under health promotion and maintenance, and system-specific and focused assessments under reduction of risk potential.

Timing

Exam mode asks 25 of the 62 in 50 minutes: two minutes each, the pace of 150 items in five hours.

Where the answers come from

Each of these NCLEX questions for health assessment names the section of the open textbook Nursing Skills its answer was read in. No review book or question bank was used.

  • Most questions turn on one thing: telling an expected finding from one that must be reported.

Questions and answers

What health assessment topics are on the NCLEX?

By the test plan: the health history and interviewing technique, targeted screening, equipment and technique suited to the client's age, comprehensive and focused assessments, and checks of peripheral pulses, neurological status and peripheral edema.

In what order is the abdomen assessed?

Inspection, then auscultation, then palpation and percussion. Listening comes before touching so that the bowel sounds are heard undisturbed, by the Nursing Skills chapter cited on the abdominal questions.

Are these real NCLEX health assessment questions?

No. The test plan says disclosing exam items is a violation of law. All 62 are our own, and none repeats or rewords an item of the exam, of NCSBN's samples or of a review book.

How many health assessment questions are on the NCLEX?

NCSBN publishes no count. Health promotion and maintenance carries 6 to 12% of items and reduction of risk potential 9 to 15%, and assessment is one part of each.
