How this list was compiled: Our own questions, written from the ICD-10-CM Official Guidelines for Coding and Reporting, FY 2027 (October 1, 2026 through September 30, 2027), published by the Centers for Medicare & Medicaid Services and the National Center for Health Statistics (https://www.cms.gov/files/document/fy-2027-icd-10-cm-coding-guidelines.pdf, linked from https://www.cms.gov/medicare/coding-billing/icd-10-codes, read 2026-10-08). The topics follow the order of the guidelines: Section I.A conventions, Section I.B general rules, the chapter guidelines for Chapters 1, 2, 4, 9, 15, 19 with 20, and 21, then Sections II, III and IV. The bank holds 22, 21, 17, 14, 11, 17, 14, 25, 13, 21 questions in that order. Code numbers and titles are from the FY 2027 code descriptions file and the Alphabetic Index of the same page (2027 Code Descriptions in Tabular Order; 2027 Code Tables, Tabular and Index), read 2026-10-08. What the questions follow

The ICD-10-CM Official Guidelines for Coding and Reporting for fiscal year 2027, in effect October 1, 2026 through September 30, 2027, read at cms.gov on October 8, 2026. Topics run in the guidelines' order: conventions, general rules, eight chapters, then principal, additional and outpatient diagnoses.

ICD 10 coding practice with cases

About a quarter of the questions are ICD 10 test cases: a short diagnostic statement and codes to choose from, settled by a rule. Code titles come from the FY 2027 code descriptions file.

A code set, not one exam

ICD-10-CM has no official length, time limit or pass mark to copy. Exam mode asks 25 questions and gives the number right.

  • Codes and rules change each October 1, so an older code book may disagree with fiscal year 2027.

Questions and answers

Is this ICD 10 practice test free?

Yes. It is free with no sign-up, and it covers ICD-10-CM diagnosis coding only, not ICD-10-PCS or any other procedure code set.

Does it give ICD 10 test questions and answers?

Yes. After each choice the right answer shows with a short reason and its guideline section, such as I.C.9.a, so these ICD 10 questions and answers can be checked in the federal file.

Is there an ICD-10-CM practice questions and answers PDF?

After a set, Print opens a question sheet that you can lengthen, with its answer key if you tick the box. The print window of your browser can save it as a PDF.

Are these ICD 10 CM practice questions from a real exam?

No. Every one of these ICD 10 practice questions is our own, written from the federal guidelines, and none is copied or reworded from a certification exam, a code book or a prep site.
