How this list was compiled: Our own questions, written to the eight training topics that 49 CFR 390.105(b) lists for medical examiners (https://www.ecfr.gov/current/title-49/section-390.105, eCFR text of October 5, 2026, read 2026-10-09). The rule gives the topics no weights. The bank holds 12, 12, 10, 16, 13, 9, 27, 13 questions in the order of the rule. Every fact was read on 2026-10-09 on ecfr.gov in the section the question names: 49 CFR part 391 subpart E (sections 391.41 to 391.49), Appendix A to part 391 (Medical Advisory Criteria, which calls itself advisory), part 390 subpart D, and sections 381.300, 390.5T, 391.2, 391.23, 391.62 and 392.3. The eight training topics

49 CFR 390.105(b), read October 9, 2026, lists eight training topics for medical examiners: the examiner's role, the work of driving, history and medicines, the examination, further tests, educating the driver, the certification outcome, and reporting. The rule gives them no weights.

Rule and advice

Sections 391.41 to 391.49 are the binding standards. Appendix A to part 391, the Medical Advisory Criteria, calls itself strictly advisory, so each question drawn from it names Appendix A.

  • Certificate periods carry the most questions: 24 months at most, 12 for insulin-treated diabetes and the alternative vision standard, less for raised blood pressure.
  • Form numbers recur: MCSA-5875 is the report, MCSA-5876 the certificate, MCSA-5850 the results sent to FMCSA, MCSA-5870 diabetes, MCSA-5871 vision.

Questions and answers

Is this NRCME practice test free?

Yes. There is no charge and no sign-up. It works as an NRCME practice test online, in any browser, and each answer shows the rule section it was written from.

What is on the NRCME exam?

FMCSA's 2012 final rule (77 FR 24104) says the test, like the training, is based on its regulations and guidelines. 49 CFR 390.105(b) sets eight training topics, from the examiner's role to reporting. FMCSA provides the test; organizations that meet section 390.107 give it.

How many questions are on the NRCME test?

49 CFR part 390 subpart D states no question count, time limit or pass mark, and this page does not guess one. Ask the testing organization you book with.

How long is NRCME certification good for?

The credential expires 10 years after issue (49 CFR 390.109). To stay listed, an examiner completes periodic training between years 4 and 5, then training and the test again between years 9 and 10.

Are these the real NRCME test questions?

No. These NRCME practice tests hold only our own questions, written from the federal rule. Section 390.107 has testing organizations keep the real test items secure, and none is used here.
